| 8200 | Enrollment Permanency Planning | Adds guidance for conducting enrollment permanency planning meetings. Clarifies time frame for the LIDDAs to complete enrollment permanency planning within 20 calendar days. Adds note to clarify that the LIDDA who completed the enrollment is responsible for completing an initial plan for enrollment into the current placement, regardless of whether a LIDDA reassignment took place. |
| 8300 | Continued Permanency Planning | Adds guidance on conducting permanency planning for persons who currently reside in an ICF/IID/SSLC or HCS residential setting. Adds clarification that any permanency plan recommendations including continued placement in an institution after the initial six-month extension must be reviewed and approved by a secondary reviewer. Adds note to clarify that updated permanency planning reviews are not required when a person moves from one eligible setting to another. |
| 8400 | Permanency Planning Reports | Clarifies the purpose of specific CARE reports. Clarifies that the LIDDA is responsible for running weekly reports to identify and track all initial permanency plans and permanency plan reviews. Adds guidance for the LIDDA on alerting the correct LIDDA if an unfamiliar person appears on their report. Adds note to clarify that a person may count toward a LIDDA’s performance percentage if the person continues to appear on the LIDDA’s report without action to complete the plan or coordinate with the correct LIDDA. |
