Purpose
- To explain what Texas Health Steps is and what services it provides.
- To inform clients about the requirement to schedule a medical check-up for a child under age 19 who is new to Medicaid.
- To give clients a toll-free number to call for assistance in scheduling a check-up.
Procedure
When to Prepare
Mail one copy of the form with Form H1122 to the caretaker of a child under 19 after certification for Medicaid.
Number of Copies and Transmittal
Give one form to each applicant.
Form Retention
No retention requirements.
