Purpose

Form 3233 is used to obtain a criminal history check from both the Texas Department of Public Safety (DPS) and the Federal Bureau of Investigations (FBI), as required by Texas Administrative Code Title 26 Section 562.31. Potential applicants for Chemical Dependency Counselor Intern (LCDC-I) registration may use Form 3233 to check their criminal history before applying for LCDC-I registration.

Fingerprint Instructions

Once the Texas Health and Human Services Commission (HHSC) receives a complete Form 3233 and fee, HHSC will send instructions and a unique code to obtain two sets of fingerprints from IdentoGo. This is needed to obtain a criminal history check from both the DPS and the FBI, as required by 26 TAC Section 562.31.
