Purpose
A currently registered LCDC counselor intern uses Form 3266 to apply for a licensed chemical dependency counselor (LCDC) license.
Procedure
When to Prepare
A counselor intern must submit this full license application, fees and other required documents once they have completed all the requirements for LCDC licensure and are eligible to apply.

HHSC must review and approve all required application materials received before HHSC issues an LCDC license.
What to Prepare – Requirements for LCDC Licensure
A counselor intern seeking an LCDC license must meet all eligibility criteria under Texas Administrative Code (TAC) Title 26, Chapter 562, Licensed Chemical Dependency Counselors, Section 562.9.
A counselor intern eligible for LCDC licensure must submit all the following to HHSC per 26 TAC Section 562.9:
- A completed, signed and dated Form 3266.
- An initial license fee of $85 that includes $75 plus a surcharge of $10 to fund one or more approved peer assistance programs.
Note: A military service member, military spouse or military veteran must complete Form 3242, Military Member, Military Spouse and Military Veteran Supplemental and submit it with Form 3266. The licensure fee is not required per Texas Occupations Code Section 55.009. - A copy of the letter from the Texas Certification Board (TCB) that shows the counselor intern passed the Alcohol and Drug Counselor exam and the score.
- Two letters of recommendation from LCDCs.
- An official transcript that shows successful completion of an associate degree or more advanced degree if you did not previously submit transcripts for an application or supervised work experience (SWE) waiver. See the transcript and waiver information below.
A counselor intern must make sure the clinical training institution (CTI) coordinator or certified clinical supervisor (CCS) submits a completed and signed Form 3262 LCDC Intern Supervised Work Experience Documentation along with the job description for the internship to HHSC, as required by 26 TAC Section 562.21(j). The form must show proof of completion of 4,000 hours of supervised work experience as a registered counselor intern.

Timeline
After submitting the required application, fees and documentation, expect to hear from HHSC within 45 days per 26 TAC Section 562.5(g).
Waiver
Applicants may request HHSC to waive the 4,000 hours of supervised work experience for people who hold a master's or doctoral degree in a counseling-related field defined in 26 TAC Section 562.1(18) and have completed 48 semester hours of graduate-level coursework. Examples of the counseling-related fields include social work, counseling, substance abuse, addiction studies or psychology.
Transcripts
Methods to send a transcript to HHSC:
- Preferred: The school emails transcript directly to HHSC at lcdc@hhs.texas.gov.
- The counselor intern requests an official college transcript from the school and includes the sealed document with their application.
