The Texas Health and Human Services Commission (HHSC) case manager completes Form 2060, Needs Assessment Questionnaire and Task/Hour Guide, to determine the individual's needs. The additional eligibility requirements are outlined under Chapter 48, Community Care for Aged and Disabled (CCAD).
