Document how the person’s identity was authenticated. This requirement applies when disclosing individually identifiable health information and when contact occurs outside the interview.
Document:
- The name of the personal representative (PR).
- The reason a parent is not considered a PR for an unemancipated minor.
- That the inquiring agency agrees to keep information confidential.
- That the information is limited to the purpose of the disclosure.
